In line with the NHS Long Term Plan, the CCG is developing with partners a new community-based offer for mental health services. Community mental health teams will be reorganised to move towards place-based, multi-disciplinary services across health and social care and aligned with primary care networks

We know that many people benefit from receiving care and support in their own home as much as possible, and this model will enable us to provide this to more people who require it.
