NHS Vale of York Clinical Commissioning Group (CCG) and York Teaching Hospital NHS Foundation Trust (YTHFT) could take home a national Health Service Journal (HSJ) award later this week thanks to Proactive Health Coaching, a telephone-based health management service that’s reduced patients’ A&E attendances by a third.

The service jointly designed by health technology company Health Navigator in partnership with the CCG and YTHT, is delivered by registered nurses and healthcare professionals through weekly telephone coaching calls, aimed to support patients with complex conditions and empower them to take control of their health.

The CCG, YTFHT and Health Navigator have been recognised in the HSJ ‘HealthTech Partnership of the Year award’ category, celebrating a number of healthcare achievements:

• 36% reduction in A&E attendances for those patients supported by health coaching compared to patients that had not received the intervention
• 24% reduction in non-elective (emergency admissions)
• 55% of patients recruited into the programme from York Hospital felt much more engaged with their care
• 84% of people reported an improved quality of life and having more confidence in managing their conditions
• successful expansion to an additional 1,800 patients

Andrew Lee, the CCG’s Executive Director for Primary Care and Population Heath said: “We are thrilled that the proactive health coaching service has been recognised for supporting people with long term conditions to improve their health, resulting in significantly reduced attendances at A&E and more importantly, making a positive impact on patients’ quality of life.

“Being a finalist in the HSJ partnership category demonstrates the strong relationship the CCG has with York Hospital and wider partners with joint aims to deliver the best care on offer for our patients.”

The CGG was the first in the country to trial the innovative AI based nursing service from Health Navigator and now 10 other CCG’s across the country are following suit.
