Health partners set for national award recognising York’s new service to reduce A&E attendances

NHS Vale of York Clinical Commissioning Group (CCG) and York Teaching Hospital NHS Foundation Trust (YTHFT) could take home a national Health Service Journal (HSJ) award later this week thanks to Proactive Health Coaching, a telephone-based health management service that’s reduced patients’ A&E attendances by a third.

The service jointly designed by health technology company Health Navigator in partnership with the CCG and YTHT, is delivered by registered nurses and healthcare professionals through weekly telephone coaching calls, aimed to support patients with complex conditions and empower them to take control of their health.

The CCG, YTFHT and Health Navigator have been recognised in the HSJ ‘HealthTech Partnership of the Year award’ category, celebrating a number of healthcare achievements:

Andrew Lee, the CCG’s Executive Director for Primary Care and Population Heath said: “We are thrilled that the proactive health coaching service has been recognised for supporting people with long term conditions to improve their health, resulting in significantly reduced attendances at A&E and more importantly, making a positive impact on patients’ quality of life.

Being a finalist in the HSJ partnership category demonstrates the strong relationship the CCG has with York Hospital and wider partners with joint aims to deliver the best care on offer for our patients.”

Melanie Liley, Deputy Chief Operating Officer, York Teaching Hospital NHS Foundation Trust said: “We are delighted that proactive health coaching service, which helps make a real difference to patients managing ongoing and complex health conditions, has been recognised in this way.  We look forward to continuing to work in partnership with them and the CCG to innovate, improve and deliver services for our patients.”

The CGG was the first in the country to trial the innovative AI based nursing service from Health Navigator and now 10 other CCG’s across the country are following suit.

Joachim Werr, Health Navigator CEO, said: “The proactive health coaching project in Vale of York has seen our technology and health-coaching adapted to best serve the priorities of the local population and health system. Making it to the final stage of the HSJ Partnership Awards really demonstrates both the commitment of the local NHS to delivering improved care outcomes across the system, and the strength of collaboration between the CCG, the Trust, and Health Navigator. It has been great to work with a forward-thinking care system, which has invested in building a service that delivers measurable benefits to the Vale of York population.”

Winners will be announced at the HSJ awards ceremony in London on Thursday 27 February 2020. The full list of finalist can be found on the HSJ website: https://partnership.hsj.co.uk/partnership-awards-finalists-2020
